Our commitment

Peace of Mind MMJ, LLC is committed to protecting the privacy of your protected health information (PHI). We are required by law to maintain the privacy of your PHI, to provide you with this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect.

How we may use and disclose your health information

  • For treatment and certification: to provide and coordinate the medical certification and application-assistance service you request.
  • For your Illinois application: to submit required information to the State of Illinois as part of your medical cannabis application, as directed by you.
  • For payment and operations: to process payment for our services and to run our business appropriately.
  • As required by law: when federal, state, or local law requires it.

Your rights regarding your health information

  • The right to request access to and a copy of your PHI.
  • The right to request a correction of your PHI.
  • The right to request restrictions on certain uses and disclosures.
  • The right to request confidential communications.
  • The right to an accounting of certain disclosures.
  • The right to a paper copy of this notice.

Business associates

We may share PHI with vendors who perform services on our behalf. Those vendors are bound by Business Associate Agreements requiring them to protect your information.
