How we help
We help you add a designated caregiver in your IDPH patient account and complete the caregiver application. You can fill out the caregiver's application yourself, or IDPH emails the caregiver a form to finish. An adult patient may add up to 3 caregivers, and each caregiver application includes its own identity and address details.
Who this is for
Patients who want to name a designated caregiver in their IDPH account, and the caregivers they choose, who complete their own registration.
The steps
- Open Manage Caregiver(s) in your IDPH patient account and add the caregiver's details.
- Choose whether you fill out the caregiver's application or IDPH emails the caregiver a form.
- The caregiver uses the login link in the IDPH email to complete their registration.
- Upload a photo or a photo affidavit, an ID document, and proof of residence if needed.
- The caregiver signs the Caregiver Consent page and submits the application.
What to bring
- The caregiver's name, address, and date of birth
- The caregiver's email address
- A photo of the caregiver, or a photo affidavit
- The caregiver's driver's license, state ID card, or passport
- Proof of residence if the caregiver's address differs from the ID

How many caregivers can an adult patient add?
An adult patient may add up to 3 designated caregivers. Caregivers can be added or canceled later with the Manage Caregiver option, and we help with the forms each time.
