Separate, optional consent

Providing a phone number does not by itself authorize SMS. An adult must separately opt in for the stated message category. Consent is not a condition of purchasing a service and is recorded with the disclosed version, source, time, and number.

Message scope and frequency

Messages are limited to requested transactional updates such as appointment, document, submission-status, or renewal reminders. Frequency varies with the requested service. Sensitive medical detail is not included in SMS; recipients are directed to the secure portal.

Charges and delivery

Message and data rates may apply. Carriers and delivery providers are not liable for delayed or undelivered messages. A provider acceptance receipt is not represented as handset delivery.

STOP, START, and HELP

Reply STOP to opt out. Reply START only if you want to opt back in after reviewing the current disclosure. Reply HELP or contact us for assistance. Opt-out is honored across transactional campaigns for that number, subject to legally required communications through another channel.
