Before you schedule

• Confirm that the appointment is for the Illinois adult patient program.
• Ask which portions can take place remotely and which require an in person visit.
• Confirm the patient package total and the state fee included in it.
• Ask which identification, residency, and clinical records are required for your situation.
• If requesting reduced pricing, ask which supporting eligibility documents to prepare.

Before the evaluation

Organize the requested clinical records and any questions for the clinician. Keep medical information out of ordinary website forms and email. Call for instructions about the appropriate document route.

If using telehealth, test your device, sound, connection, and a private space before the scheduled visit.

Before submitting the application

Review your name, contact information, documents, and application details. Keep control of your own account credentials. IDPH’s current patient resources explain access through ILogin. Staff can explain administrative steps without replacing your required review and authorization.
