Explain whether you are asking about a caregiver for an adult patient or a minor patient. The published patient package table does not quote an additional caregiver registration. Confirm the assistance offered, required documents, and total cost first.

State fees are different from service prices

IDPH publishes designated caregiver fees of $25, $50, or $75 for one, two, or three years. Patient and caregiver combinations have their own state totals. Consult the current IDPH fee table and confirm the applicable service quote with our team.
